Form name
The Script Wizard must create a form for containing the text box used to display the access information. The form must have a unique name, to distinguish it from any other forms in the document. Enter a name for the form here, or simply accept the default name generated by the Script Wizard.
Form field name
A text box is a type of form field. All form fields must have names to distinguish them from one another. Enter a name for the text box here, or simply accept the default name generated by the Script Wizard.
The form name and the form field name must both begin with a letter and may only contain letters, numbers, and underscores (_). Spaces and symbols cannot be used.
Information type
Visitor’s name
Creates a script that opens a dialog box when the current document is loaded in a visitor’s browser. The dialog box prompts the site visitor for information, such as his or her name. The information supplied by the visitor then appears in a one-line text box.
Last modification date and time
Creates a one-line text box that displays the date that the current document was last modified (saved).
Visitor’s IP address (Netscape only)
Creates a one-line text box that displays the visitor’s IP address.
Visitor’s host name (Netscape only)
Creates a one-line text box that displays the visitor’s host name.
